ACCESS TO CARE
This program provides primary health care to adults who meet the program guidelines. In this program each participant is assigned a primary care physician who can be seen for illness. Most lab tests and x-rays are covered, as well as prescriptions, with a small co-pay, if ordered by the primary care physician. All Orland Township residents may call (708) 403-4222 to schedule an appointment to apply for this program.
Services Include:
Visits to your Access to Care doctor for illness.
Most lab tests ordered by your Access to Care doctor.
Most X-Rays ordered by your Access to Care doctor.
Most prescription drugs prescribed by your Access to Care doctor.
Services only covered by PRIOR APPROVAL:
Mammograms and Ultrasounds
Services not Included:
Physicals
Hospital Care
Immunizations
Prenatal Care
Well Baby/Child Care
Emergency Room Care
Family Planning/Birth Control
Over the Counter Drugs (except insulin and diabetic testing supplies)
Sexually Transmitted Disease Diagnosis and Treatment
Care provided by a non-primary care physician specialist*
Care provided by an obstetrician or gynecologist.
*Please note that Access to Care does not pay for specialty care. If you are referred by your doctor or by Access to Care to another doctor or facility for specialty care, you must pay for the services as required by the doctor or facility.

Services, lab tests, x-rays and/or prescriptions ordered by a doctor other then your Access to Care doctor or provided by any lab station, radiology site and/or pharmacy other than an Access to Care contracted facility WILL NOT BE COVERED!
