WELLNESS SERVICES
Please visit the Orland Township office at 15100 S. 94th Ave., Orland Park to select a physician for school, camp, or sports physicals; a dentist for a screening examination, cleaning or x-rays; an ophthalmologist or optometrist for a vision examination; a chiropractor for an examination; or a podiatrist for an examination and debridement, from the list of those participating. The fees charged represent discounts of as much as, if not more, than, 50% per medical service. Payment for the service may be made at the Township. Appointments must be scheduled with the physician by the resident.
(Please note that the physical examination may not be used for an illness. Proof of residency will be required at the time of discount voucher purchase.)
